Defeat the Mantis.
You will reach the Mantis at the end of level 5 the catacombs, You will enter a portal then approach a large Mantis statue with a key nearby luring you in. The statue will come to life and attack you. The Mantis will jump around and try to get in your face to attack you, the Mantis will also cling to the walls.
Achievement is worth 100 Gamerscore.
