This is story related and cannot be missed. Acquired near the end of Chapter 11. After going to up the elevator Rot will say a few words and the achievement will pop seconds afterward.
It is also good to note that when you are going underwater to get the stash, the controls are much slower as you are walking against a current of water, don't freak out at this as it is supposed to happen, lol.
