This is a rather "long" achievement, but you'll get it after a few solid hours of playing through Survival and Marathon mode.

Simply put, every time a set of hexagons disappears, either because you turned the pieces or because they fell and became a combo, it counts as a combo.

Just play a lot and it'll come over time. This does count all combos over each game type.
