Reverse a decision you made on in an upgrade grid.
This is one of the easier trophy/achievements to get in Destiny.
You unlock upgrades for your character's subclass and items by gaining experience. You'll notice on the upgrade menu that some upgrades are below others, which means that they are related to each other, and they replace one another. When you replace one upgrade with another, you'll obtain the trophy/achievement.
Your first opportunity to do this will most likely be when you reach Lv7 and you unlock a different grenade-ability for your subclass.
