Once save is loaded on console, all you have to do is go up to Teresa and take her hand she'll go through a little speach then give you the option to select 1 of the 3 endings.

Choose the one you need, then after the achievement pops quit the game to the dashboard (before it saves) then reload and get the other ones you need.

If for some reason the game saves on you before you quit to the dash, just reload the safe file from the computer to your MU.
