I have done 5 out of 6 war stories but the 6th one is nowhere to be seen? Have i missed something or whaaaaaaat

Just done both and still not there, do i have to let the credits finish? I stopped after 5 mins. Also is there a certain amount of codex entries and field manuals i need to get?

I'd make sure you did all the missions then. I have no idea, possibly it's bugged? or maybe you just missed something. Codex entries and field manuals should have nothing to do with it.
