When Michael cast lucifer down to the cage it was said he "fell" does this mean the presumed battle in Maryland? I'm confused as to was it Lucifer AND Michael in the cage or was it just Lucifer, then Michael joined him when Sam took control and pulled him along when the Winchesters used the horsemen rings?
