Twitch avatar + Stream overlay

Hi!
Im looking for Twitch avatar + stream overlay for my stream.
My twitch name is Nausku, and would like the avatar be something 'handwritten' style font with white/blue/black background and/or font.
For the overlay id like something abstract with same colors (white, blue, black)
Id like bottom line or something like that with abstract style lines.
Would want 1 bar that has 'Latest follower' and the latest follower rolls there.
Also it would be nice if it could have 'health bar' and 'ammo bar' on transparent. (bc of playing CSGO)
My stream resolution is 1280x720 (stretched)
