3) Uncertain, It's an ATX Mid Tower though. Lots of room for what's inside.
4) Not sure, fan just stopped running one day. No sounds, just doesn't move. (I also tried to move it with my fingers and it's much harder to move compared to my GPU/Case fans.)

(Note: The fan cannot be any larger than my current fan. As-is the fan hardly fits. 3 or 4 pin.)

If the fan stopped working, it doesn't mean the cooler is broken, just the fan. Could just replace the fan probably, or buy another of the same cooler if it was doing the job for you. I don't know what case you have so I can't really suggest something in case it's incompatible. 212 EVO is always a good shout for a budget cooler, if it fits your case.

Push the boat out a little and go for the Corsair H55 Water Cooling, £50 very good value for money because I know from experience the FX-6300 can run extremely hot under stress.
