I was just online with XBLogon, then switched plugins, and put my KV on the HDD
Then I connected and got console banned. I swear I did. I'm not mad, lol. Stuff happens

Is there anything I'm missing? I put it as the first plugin on the root of my HDD, then put the KV on my HDD's root.

Yes I did save the launch.ini and yes I did reboot.

Dude I don't know who you are but you obviously don't know much about this. In IDA, you search for "xosc". From there, reverse and you'll see that it's hooked by XexLoadImageFromMemory. XOSC is public, the Titan and XBLS sources leaked.

You aren't missing anything. All you need is the plugin. I'm not sure why you got banned instantly, that has never happened to me.
