I get into online gameplay then it closes and an alert appears saying

The game was restarted due to a problem with the storage device where the game data was installed.

Please replace the storage device and re install the game data is issues continue to occur.

Can anyone help?

I have tried deleting from the storage and re loading the disc 1 it works for a few minutes then crashes out again.

Should I format the HDD?

Try putting it on a different storage device, not sure. Say if it works.

I have cleared the cache also, and still not worked.
I have another account which I have saved game too, which I am now deleting and trying to re install from disc one again.

I have however noticed that on disc 2 I have a scratch which may be causing the game to crash? Not sure?

If not then I shall try purchasing another game today I guess as there is no problem when loading disc 1 onto HDD nor downloading the update.

try that but you don't need to purchase a new disc, just get it cleaned and buffed out for about $5
