So I was just screwing around and something worked and I was able to use my custom game. Here is an explanation of what I did:
(Patched: UNKNOWN) (Credit: xCULTx)
Set up your custom game with per-team set.
Save your game.
Now next I backed out to the multiplayer menu and selected CODTV.
Then select a clip and LOAD it normally. I used a longer clip, don't know if it is revelant.
Now go to your player channel and click your custom game.
I got annoyed at this point and threw my controller (Don't do this) but I noticed my analog stick was held down and the highlight on the screen was changing rapidly. (Yes/No were both light orange)
So hold your right stick down and let go. Do this until it lands on YES.
Now press A+B.
Done.

Please test and see if this works for you.

Secondary Method (Patched Status: UNKNOWN) (Credit: Elite Chaos)
1- Start by joining a party. (It can be your own.)
2- Then Go to Custom Games
3- Make a edited gametype (Do not edit amount of teams, Health and Damage settings, or Bots)(Sorry about no Bots but the game can't use bots online.)
4- Go to preset classes
5- Change it from global to Per-Team (Some Gametypes don't have this. If not then its fine.)
6- You have to have Amount of selections on 10 (Some Gametypes don't have this. If not then its fine)
7- Now change the map to Nuketown 2025
8- Next you have to go to change game settings
9- Then save that gametype.
10- Now go Public Match, Find Match, Whichever gametype you edited, then leave that lobby.
11- Go to Cod TV, then to friends feed, recently shared.
12- Then select the first video and hover over yes, (Do you want to go to theater)
13- Now you have to press A and B at the SAME time.
14- If done correctly then you should be in a online match.
15- Now you have to press Y (Shows Friends)
16- Press RB to go to Xbox Live Party.
17- Click on your GT, Then on Player channel.
18- Go to Custom Games.
19- Scroll over yes, Then press A and B at the SAME time.
20- It should put you into Custom Games
21- From Custom Games go to Change game mode
22- Click on Saved, Then on the edited gametype.
23- Now leave that lobby
24- Now go Public Match, Find Match, Whichever gametype you edited, then leave that lobby.
25- Go to Cod TV, then to friends feed, recently shared.
26- Then select the first video and hover over yes, (Do you want to go to theater)
27- Now you have to press A and B at the SAME time.
28- If done correctly then you should be in a online match.

You need to be the host for this to work
