Hello i have the original GTA IV disk and also the lost and damned DLC. i was thinking about buying the ballad of gay tony dlc i was wondering will i then be able to play with my friends as they have episodes from liberty city?

Yes, but you will have to load the liberty city gameplay/save in the start menu

Each GTA IV addon has it's own game mode and multiplayer.

If you want to play on the liberty city multiplayer, then you will have to press start, go all the way to the right and click on load, then you will just click on a liberty city game save and it will transfer you to the liberty city GTA. Then you can go to liberty City's multiplayer from there.
