Is prestiging the only way to get them

No. U can also get them if u prestiged in cod 4, WAW, MW2 and black ops. 1 per prestige in each of those. Hope this helped

EXCEPT you only get 1 if you prestige any amount (If you get 6 prestige in cod4 then you will only get 1 token). Don't quote me on that I'm not 100% sure.

yes you get one token per game. but people are getting WaW lobbies and getting a token and then deranking them selves and getting another token.

This is correct. I've prestiged in all COD (MW, WaW, MW2, not black ops) and I started with 3 tokens so that statement makes sense.
