you will need to access this website:

Once you are on this website, you will need to scroll down till you see a button saying "GET DAY ZERO CONTENT HERE"
Click this and you will need to login with facebook or enter your details from your call of duty account. If you haven't got either of these you will need to make an account. Then you should click the login button and you should see on screen the following message:
THANK YOU FOR PURCHASING THE CALL OF DUTY: ADVANCED WARFARE DAY ZERO EDITION OR COLLECTOR'S EDITION!
To the right hand side there will be a check box saying "Please click here if you are entitled to this content and did not receive it." You will need to check the box so that a tick appears and click the submit button. The DLC will be with you within 24 hours as it says on screen.
