Hello there Optus here bringing you Tutorias on The Ranks!
5+ KD - 2 Bones
10+ KD - Skull
71.5 KD - Skull with dagger
140-200 KD - Skull with Shotgun
To get the blue eyes you need to get 5 Tally Marks on your emblem.
To get tally marks you need to play everyday and get good stats!
To find your kd Divide Your kills by downs Ex. 15873 Divided by 336 = 47 KD so i need 24.5 more to get 71.5
Some easy Tutorials on getting bunch of kills with 0 Deaths :

I earned my Skull with Dagger at around 62 Kills/Downs.
Also i've noticed that you get a tally for every day you play that you get above round 10

I am sorry to say that one percent of what you posted is incorrect. At least, in my case it is. I had around 17k kills and maybe 250 downs (Yes I know thats decently bad but I really do not care) and that was around 68 Kill/Down and that is when I have received the Skull with the Knife. I thought I would just inform you, but it is different for everyone.
