to see if your rgh is flashed pop in a burned iso if it reads its flashed if not its not. and make sure your disks are i decent condition

If your out of memory or if your disc is dirty or even if your drive has a dirty laser or old laser it may be difficult to install games by ripping them with xex menu or FSD.

I know COD4 has a hardtime ripping but i havent had any problems with newer games.

You can check if its flashed by insterting a burned game if it says play dvd or unsuported you got a stock FW installed still..

I had this same exact problem. What i did was connect my jtag to my router, and use flashftp to exctract the game from my jtag, to my hard drive on my computer, then copy the game to my jtag from my computer. It takes longer but it worked for me.

You know I never once got a game to copy via xex menu, or freestyle dash they always seem to stop before it even halfway finished. I tried a dozen different games, tried to copy via USB, with a original disc, NEVER worked for me.

I recommend jtag content manager 2.2, one click and it puts games in the right folder and takes about 10 minutes for a a average game.

Also with freestyle dash it has a built in FTP, so you can just drag and drop a folder to your Xbox hardrive, I'd also recommend the latest sdk, works like a charm.
