This trophy requires you to have 55 pennies on you and is not cumulative. You may not get this early in the game without actively trying. An easy way to do this is to find a "2 of diamonds" which will double your current coins. If you are past Mom's heart, you will probably unlock this without trying. You can also play the Slot Machine that is found randomly and in the arcade, on occasion it will reward you with "The dollar bill" which will grant you 99 coins instantly but it can be a rare occurrence. Personally I don't think Cain is all that great. His range is so-so and his other stats are about the same. The redeeming factor is he starts with a key, which can be useful when the game does not give you a key in the first few levels.
