Your team incapacitates three Survivors within five seconds in the Crash Course campaign.
Set this up the same as for "Truck Stop". The players have to be incapacitated within five seconds of each other. You can boost it with friends, by yourself with a controller or in a public match. You can get this online or offline.

Boosting this would be your best way to get this. I believe I did, you have to be pretty quick to do it and they'd all have to be on low health prior to the attack.
