Yah it's a pre built, got it 2 years ago so most stuff needs to be upgraded.

The PSU is a 500W. ATX 12V Model: 500UB is what it says on the PSU.

I'm assuming it's the one I found online made by CiT; it's not good, and would most likely need an upgrade.

So for about 300 I would need to buy myself a new graphics card, PSU, Cooler. Anything else? I should be able to use my current 1TB hard drive right? I may put 100 extra if needed.

It's a little over budget, but worth it. New PSU that will do fine, new case since you have a pre-built, case could be poo. You might be fine to use your current case, but it'd still be worth upgrading IMO. If you don't already have an aftermarket cooler, I'd add one at some point. You're not even using a 1080p panel, so the GTX 950 will be more than adequate. You can save up for a nice 1080p panel or something, and the 950 will still do fine.

Yeah, everything else you have will be compatible so you'll be fine. If you need to save a bit of money, you could get an R7 370 instead of the GTX 950. You'll be able to XFire your APU with the 370, but the 950 is only 10e more than the 370, and is a much better GPU.

Your displayed temperatures definitely don't seem accurate. An aftermarket cooler wouldn't hurt, but it's not necessary. If you can spend the extra 40e for it, go ahead.

I'd be getting this cooler, instead of the 212 EVO though;
