GTX 760, Should be a lot better than that 660, would recommend looking up some benchmarks or videos ion the 760 though to make sure it's good for you, but for 200 I cant think of anything else, it can also be powered by your PSU easily.

OR

AMD R9 280, I Have the 280 and its a decent card, I have mine running off a 500W PSU so yours will power it no problem & again I'd suggest looking up benchmarks and reviews just to make sure it fits your needs.

ehm, depends on what you want it for, with a 760 or 280 you want a 1080p monitor I guess, or maybe a 1440p monitor but you wont be getting as good FPS.

If you're gaming a lot you want something with a high refresh rate but since most monitors are at least 60Hz[Means monitor can put out 60FPS] it doesn't really matter, i'd just get whatever size you want which you can afford.
