i disconnected everything from my computer, let everything cool down then plugged everything back in, firmly and securely. also have restarted it and also reset everything to defaults, but still it happens.

I honestly am no help then. One last thing is that if you hear any constant beeping originating from the graphics card, you hear the fan running at really high rpm's, or your graphics card does not even turn on at start up then you need to replace it.

It won't be his ISP, it will be his graphics card, graphics driver, lead, or monitor.

The problem sounds like the graphics card is having an issue communicating with the motherboard. Try a bios update maybe or try moving the GPU into a different PCI Express slot. Is this a custom build or from a manufacturer?

Is your cpu cooler fitted on properly?
It may be getting too hot when your multitasking?
My fps were badly dropping and i ddidnt know why, turns out my cpu cooler wasnt attacthed properly lol.
Download a cpu temp program, i use core temp. If its not that, im afraid i cant help.

it's a custom build.

Did you try a bios update? If it's a custom build the GPU might be damaged from static electricity try test it in another computer.

i built this computer about 2 years ago and i have the latest drivers for my mobo/gpu.

Try the GPU into another PC if the same error occurs than it means it's from the GPU itself or if you have a spare GPU try it in your computer and see if the issue persists.
