Correct. Designs are either pre made or just chosen from a few previous made suggestions.

Horizon 3 also has the option to buy cars from the auction house.

You can make paints that go into really good detail and there is also an auction house on Forza Horizon 3 where you can buy other players cars

Well it all depends on personal preference and play style if you want to cruise on clean surface roads and have the power to change your car whenever you want and feel like your playing a story game, I suggest horizon 3

On the other hand I would just wait for Motorsport 7 as I tend to drift and I feel as Motorsport offers a whole lot more variety and customisation for me but it all comes down to what you wanna play

Motorsport = Track Racing
Horizon = Freeroam
