There are a couple of ways to help this situation.

1) Make sure your Vibrator switch is turned off.

On the side of your phone the little line sticking out of your phone above the volume tabs make sure thats up. That may fix the problem if its down on vibrate.

2) Go to Settings,
Music,
Then turn on sound check. This may work.

Final and last thing I can think of.

3) Go to Settings,
Go to General,
Scroll down to Accesibility,
Scroll down to Assistive Touch,
And Check that on.

These may fix your situation. Hope this helped!

Is it jailbroken?

If so, I have had a friend do this before.
Alot of tweaks will install Activator, to allow you to activate things (Such as slide top bar for SBSettings etc)

If you have assigned something on Activator to the Vol Up / Vol Down keys, then the volume will not change unless the app you assigned is open.

Easy way to check volume without buttons is open app switcher (Double tap home button) and go all the way to the left to see your volume bar

Honestly, This happened to me 2 days ago. Just blow into where you plug in the sync cable.

It happens from the build up of dust

Blow into where the charger goes, there may be dust or a liquid may have gotten in there. This tricks the iPhone into thinking it's on an iHome where the volume is not adjustable via the iPhone itself.

(I Have this problem permanently because I got water in mine)
