This tutorial is on unlocking the head for the Mechromancer called "XXX".
You need to of completed all of the story missions to recieve it.
Step 1, travel to Thousand Cuts, then Terramorphous Peak
Step 2, Summon terramorphous and kill him
Step 3, check the loot, if there is no head, save and quit, then resume
Step 4, Kill him again, keep killing him until he drops the head
