Defeat Moloch without taking damage.

This achievement should be fairly simple to do once you have completed Apocalyptic difficulty and/or The Arena achievement. With a gear score of 1300+ you can do this on Casual difficulty and melt him with Strife. Keep to the edge of the arena and be extra careful when the pink crystals come out. Each third of his health you take out, he'll increase his sword combo to become longer as well, so be sure to dash as far away as possible. Use clones when you need him to focus on something else for a moment, and hit your ultimate as soon as is becomes available. You can't take damage in this form, so you'll end the fight at that point.
