Defeat the grub queen in The Maelstrom.
Chapter 12: The Maelstrom has three different hives that will spawn small grub enemies for a decent amount of time. You will need to kill every grub that comes out of all three hives (marked on the map below) and once the final hive has been depleted of enemies, the Scarab Queen will spawn in to attack you. Kill this mini-boss to unlock the achievement.
