Reach "The End of Fire" ending.

When you reach Lothric Castle (after the Dancer fight - then climbing the ladder) go up the stairs to the left to reach the Consumed Garden. Kill the boss here and go into the room behind him, then smack the illusionary wall behind the chest at the far end. Enter to reach the Untended Graves area.

Proceed through here and beat the boss, then head to the altered Firelink Shrine. Go to where Irina would normally sit (as you enter head to where Andre is, then go right and follow the corridor to a dead end) and hit the wall here. Grab the Fire Keepers Eyes here, then go back to the regular Firelink and give them to the Fire Keeper there.

Proceed as normal and kill the last boss, once he dies look for the Fire Keepers summon sign BEFORE touching the fire. Let her do her thing (do NOT attack her) and you\'ll get this ending.
