Complete 'Just Like Old Times' and 'Endgame' on Veteran difficulty.
Make sure you fire your gun at all times and just dodge everything, I died several times trying to get to the thin cave, and when you get to the open river just shift right and left at all time so nothing gets you, it's all a bit of trial and error so it may take a few tries, and then at the end of the boat driving you fall of the edge, then the rest is plain sailing, just follow shephard.
Achievement is worth 25 gamerscore.
