Early on in the game, after you help Butters, you will go to Kupa Keep and Cartman will ask you what your name is. Whatever you put in will result in the game saying you entered the name Douchebag. Simply say yes and then yes again to unlock. If you press no at all, then you will need to start a new game and do it again.
