Open Your Eyes - Use psychostimulants.
Press [Triangle] to enter the menu and use the "Psychostimulants" just below the Handgun.

Duck If You Love Life - Avoid Jack's scissor attack by crouching.
Go down into the "Processing Area" and to the Morgue (Press "Touchpad" to open map)
Run around to the other side and interact with an item attached to a chain to start the Jack fight.
Lay into to him with shots until he makes a move for his giant scissors.
Now duck with [R3] right as he's about to attack. (Press [R1] if you need to heal)
