Changes since surf_combine_extended2:

-Added my name near spawn
-Changed glass in spawn to combine glass
-Added 4 level
-Added Win room and fight arena
-Fixed exploit where you could skip parts of the first level.
-Fixed teleport triggers at bottom of map
-Changed and fixed water and water textures (reports of death after teleporting form bottom of map)
-Added env_cubemap to more appropirate surfaces.
-Added Terrorist surf spiral (instead of CT and T sharing the same spiral in the first level)
-And some more minor changes
