This trophy unlocks when you obtain 4 soul hearts, these are the blue hearts that are found through various means.

Most commonly:

- If you are going through the game and see a rock with an "X" on it that is tinted differently, destroy it.
- It will commonly drop 1-2 soul hearts, bombs or keys.
- The shop will often sell 1 soul heart.
- Chests and items will give hearts as well. "The rosary" will reward players with 3 soul hearts, while "Squeezy" will drop 2 hearts.
- The card - "The Hierophant" will drop 2 soul hearts.
- The pill - "Balls of Steel" will add 2 soul hearts
- Lazarus is about average His special is upon death he will revive, however it drops all red hearts to 1 and is not extremely useful.
