If You have done the clantag and gamer tag check down corretly, i think i might be when you were burning the disk you must of moved it, this is caused by if you were useing a laptop you might of bumped the laptop or moved it and i caused the disk to move, same thing if you were useing a laptop, or i would highly recommend that you try burning the disk again, or it could be the patch, maybe its the leak patch and someone has changed the contolls in the patch.
